In December 2023, we headed to Echuca, Victoria, for a series of localisation workshops — taking the findings from our national co-design process and working with the local community to adapt the hVMC model to their specific context.
We met with approximately 20 local stakeholders from Echuca Regional Health, including clinicians, allied health professionals, medical specialists, and the Executive Director of Community Services. Joining them were representatives from the Primary Health Network (PHN), dementia advocates, and Aboriginal services.
Together, we discussed the co-design findings and worked collaboratively to localise the model to suit Echuca's unique geography, community, and service landscape. This process resulted in a localised version of the hVMC model — tailored to the needs and strengths of the Echuca community. Check out the model below.
