Your care journey

The Hybrid Virtual Memory Clinic helps patients, families and carers understand the assessment process, local availability and support available along the way.

What to expect

After your doctor sends a referral, our team will be in touch to check your eligibility, book your appointment and explain each step. You’ll be supported from your first call through to your appointment, results and follow-up care.

Your next steps

Step 1

We will call you

Your clinic guide will call you after we receive your referral from your GP.

They will check you are eligible for the clinic, book your appointment, and explain what to bring and how to get to the clinic.

Step 2

Meet at the clinic

On the day of your appointment, your clinic guide will meet you at the clinic.

They will take a few measurements and help you complete any outstanding questionnaires.

They will set you up for your appointment and stay close if you need anything.

Step 3

Your appointment

On the same day, you will meet with a specialist (by video or in-person).

They will ask about your memory and health, and guide you through some tasks.

Your clinic guide will make sure everything runs smoothly.

Step 4

After your appointment

After your appointment, you might be asked to complete further tests - such as a brain scan or blood tests.

Your clinic guide will explain what’s needed and help you organise them.

Step 5

Feedback appointment

A week or two after your initial appointment, your clinic guide will arrange a feedback appointment (in person or by video) with a clinician.

The clinician will explain your results and provide a personalised support plan. Your GP will also receive a copy.

Step 6

Follow-up

If needed, your clinic guide will arrange a follow-up with the clinician - this might be a phone call, video call, or in-person visit.

This usually happens up to six months after your first appointment.

It’s a chance to check in, see how you're going with your support plan, and adjust anything if needed.

Step 7

Finishing up

Once everything is complete, you’ll be discharged from the clinic. You and your GP will receive a final letter with your results and next steps.

Clinic locations & referrals

The clinic is currently available through Orange Health Service, with more locations planned from mid 2027. Referrals are made by a GP or health professional.
