The Hybrid Virtual Memory Clinic is currently accepting referrals for suitable patients in Orange, NSW. Referrals can be sent directly to the Orange Health Service intake team.

Preparing your referrals

Who can be referred

This service may be suitable for patients who:

  • $
    are concerned about their memory or thinking
  • $
    are able to travel to Orange Health Service
  • $
    are aged 40 years or over for Aboriginal and Torres Strait Islander people, or 50 years or over for others

What to include with the referral

To help the clinic team review the referral, please include relevant clinical information and any recent investigations where available.

    reason for referral, including the patient’s memory or thinking concerns
  • $
    relevant medical history and current medications
  • $
    recent blood test results
  • $
    brain imaging from the last 6 months, where available
  • $
    patient and carer contact details
  • $
    any access, communication or support needs

Training & events

We’re planning new opportunities for GPs and health professionals to learn more about the Hybrid Virtual Memory Clinic, dementia care and post-diagnostic support. More details will be shared as training dates are confirmed.

Upcoming training opportunities include two CPD-accredited GP events later this year, followed by COPE training in 2027.
